Integration of Community Social Services Organizations

Hope that the Challenge considers "integration with existing social services organizations" as an important ingredient. What the ACA is all about - buying an air conditioner so that 75-year-old Mrs. Jones doesn't keep showing up in the ED with heat-related complications. PLEASE don't just focus on medical care amid growing evidence that leveraging social supports provides "bigger bang for the buck."
